Parkway community vaccine clinic
5/12/2021 -- Medley Pharmacy will host a COVID-19 vaccine clinic at Parkway Central Middle School on May 17. They will be providing the Pfizer vaccine at this clinic. The Pfizer vaccine is currently available to ages 12 years old and up.
This event is open to the entire Parkway community including staff, parents and students ages 12 and older. Students under age 18 will need to have a signed parental consent form to participate.
The event will be held at Parkway Central Middle School (471 N. Woods Mill Road, Chesterfield, 63017) on Monday, May 17 from 1-7 pm with a return visit for the second dose on Monday, June 7.
Everyone must bring the following items to the vaccine clinic:
- A signed consent form (even if this is your 2nd dose); Click here for the consent form
- A paper printed copy (front and back) of health insurance card (even if this is your 2nd dose)
- Driver's license, if applicable
Staff who are interested in attending should work with their supervisor if scheduling an appointment during work hours.

Monday, May 17 is a virtual learning day and students wishing to participate in the vaccine clinic during school hours will need to provide their own transportation. This will be an excused absence and teachers will work with students to make up any learning missed during their vaccination appointment. Students who already participated in the April 26 event can come between 10 am and 1 pm at the same time they attended on April 26. No new appointment is needed.
