• Flu Shot Clinic for Students

    Southwest Middle School will offer a voluntary “flu shot” (influenza vaccine) clinic on Friday, October 18th, from 12:30 to 2:45 p.m. The influenza vaccine is one of the best ways to prevent getting “the flu” - a respiratory viral illness that can cause mild to severe illness. The Visiting Nurse Association (VNA) will administer the vaccine to those who have submitted the required consent. 

    If you want your student vaccinated during this clinic, complete the authorization form (see attachment below). Please complete a separate authorization form for each student to be vaccinated. Call Nurse Jacobs if you need a paper copy of the authorization sent home with your student. Return the completed authorization with a copy of your health insurance card to Nurse Jacobs by Thursday 10/10/24

    There is no cost for a student to receive the vaccine if they have either Medicaid, no health insurance, or any of the following health insurance coverage: Aetna; Anthem - Blue Cross and Blue Shield (no EPOs); Cigna (no EPOs); Coventry; Healthlink; Humana; Medica; Tricare; United Health Care (including Allsavers, Golden Rule, UMR and GEHA); Medicare Part B. If a child has health insurance but not one of the above plans, the cost is $24.00 for the quadrivalent flu shot or $27.00 if you need the preservative-free vaccine. Checks should be made out to the VNA or you may send the exact cash amount; no change is available.

    The authorization form (see the attachment or your student can pick up a copy from the nurse’s office), a copy of your health insurance card, and payment, when applicable, must be turned in to the nurse by Thursday, October 10th so that we can give the VNA an accurate count of how many vaccines we will need.
