Health Information

  • IMMUNIZATIONS
    Students are required by Missouri State Law to be current with immunizations.  Immunization requirements for the school year 2024-2025 can be found at https://health.mo.gov/living/wellness/immunizations/pdf/2025-school-requirements.pdf

    Immunizations may be obtained through your health care provider or through the Department of Health Clinics.  Students entering 12th grade must complete the Meningococcal Conjugate Vaccine (MCV) prior to the start of school in August.  Two doses of MCV are required unless the first dose was administered to a student who was 16 years of age or older, in which case only one dose is required.  One dose must be given after the age of 16.

    If your child receives any vaccinations, send a copy of the completed immunization form to the school nurse, Lois Burch, c/o Parkway West High School, 14653 Clayton Rd., Ballwin, MO 63011.  Please send immunization information before the first day of school.  Immunization forms may also be sent by fax: (314) 415-9530 or by email: lburch@parkwayschools.net

    MEDICATIONS
    Students in grades 9-12 may carry their own over-the-counter medication in the original container with a permission note signed and dated by parent/guardian requesting self-administration.  The school nurse does not stock any medication for general student use. Disbursement of Tylenol or other medication to students, without written instruction from a physician, is not permitted.  Prescription medications must be kept locked in the nurse’s office, in their original container, with a pharmacy label or physician's prescription attached and a parental consent note.  Forms for the administration of medication at school can be found here.

    PHYSICAL EXAMINATIONS
    Physical examinations are recommended each year.  If a student intends to participate in athletics, a physical is required every other year before participation.  The Athletic Participation Packet (Privit) must be obtained from the Activities Office or downloaded here.  The completed sports physical is to be turned into the Activities Office only.  One week prior to practice/tryout, students will pick up a practice card from the Activities Office.

    HEALTH CONCERNS

    1. If there are health problems that may impact a student’s activities or attendance at school, please provide this information in writing to the nurse.
    2. Children who are ill with a fever of 100 or more should stay home for at least 24 hours until they are fever-free without the use of fever-reducing medication.  If they have diarrhea or vomiting, they should also stay home until they are symptom-free for 24 hours.
    3. Students who become ill at school are encouraged to come to the nurse’s office to call a parent/guardian.  The nurse will assist with whatever course of action is needed.

    PLEASE CONSULT THE SCHOOL NURSE

    1. If your child is hospitalized or is under a physician’s care that may impact attendance or activities at school.  Provide a copy of the hospital discharge or other medical information to the nurse or counselor when the student returns to school.
    2. Regarding any contagious disease such as chicken pox, mono, strep throat, or lice.
    3. With any medical information you would like to share.
    4. Regarding any prolonged illness that results in absence longer than one week.
