• On Tuesday, October 8th, the Dental Safari will be at Northeast Middle. If you are interested, please complete this form. This is a mobile dental corporation that travels to schools to provide dental wellness checkups. They are full licensed professionals. All children are eligible. 

    • Children with a State Medicaid Identification number
    • Children with private insurance (provide information)
    • Child with cash payment (cash or check) -no Free/Reduced Lunch ($75 reduced fee includes exam, cleaning, fluoride, sealants as needed)
    • Provisional (Need-Based) Services – No Charge – qualify for Free/Reduced Lunch and No Insurance
