We're excited to share a new, simplified way to submit your student's summer medical forms, including but not limited to immunizations, physician orders, physicals and health care plans.

To streamline this process, we've created a Google Form that's easy to use. Please use this form to submit all health documentation for your Henry student.

Important: Please complete a separate form for each student.
