Parkway vaccination booster event for eligible staff and students (ages 12 and older)

1/18/2022 -- All eligible staff and students ages 12 and older may participate in a vaccination booster event Tuesday, January 25 from 3 - 7 p.m. at the Central High commons.

Staff members may sign up their children to receive a booster. An appointment time should be made for each individual who will be getting a booster vaccination that day.

Individuals are eligible for a COVID-19 booster five months after they have completed the original COVID-19 vaccination series.

Individuals will need to bring:

  • Proof of insurance
  • Previous Vaccine Record/Card
  • Completed consent form (Minors MUST have a parent signature) The consent form is in the sign-up link and there is also a link below to the form.
