Religious Exemption - What Parents Need to Know
Religious Vaccination Exemption ReminderUnder the code of state regulations, students can be exempt from vaccinations due to religious beliefs, as follows: "This exemption must be provided on an original Department of Health and Senior Services' form Imm.P.11.A, and shall be signed by the parent of guardian and placed on file with the school immunization health record." This form may be obtained by contacting a medical provider, local public health agency, or the Missouri Department of Health and Senior Services' Bureau of Immunization Assessment and Assurance.
